Use the Word file to fill in on a computer and print; use the PDF to print and fill in by hand in blue or black ink. The samples use fictitious data; blue text marks what you fill in.

General tips
- Write foreigners' names in CAPITAL LETTERS, in the same order as in the passport.
- Put a slash (/) in any item that does not apply – do not leave it blank.
- Use the new forms: files prepared from 1 July 2026 use the NA forms of Circular 70/2026/TT-BCA.
- Foreign documents in the file must be legalised (or apostilled where accepted) and have a certified translation, unless exempt.
