How to Fill In Form NA5 (Visa and Temporary Residence Extension)

Form NA5 is the application used inside Vietnam to request a visa or an extension of temporary residence. It is the single most common immigration form here, and since 1 July 2026 only the template issued with Circular 70/2026/TT-BCA is accepted.

Who fills it in

The foreigner completes the personal details; the sponsor — a company or a relative — signs, seals and takes responsibility for the application. Company sponsors must already have form NA16 on file.

Section by section

  1. Personal details: full name exactly as printed in the passport, in the same order, plus date and place of birth and nationality. Transliteration differences are a frequent cause of rejection.
  2. Passport: number, type, issue and expiry dates. The passport must stay valid at least 30 days beyond the requested period.
  3. Current status: the visa symbol and the expiry of the current temporary residence — copy it from the stamp, not from memory.
  4. Address in Vietnam: must match the temporary residence declaration filed by the landlord.
  5. Request: what you are asking for — new visa or extension — the symbol, single or multiple entry and the duration.
  6. Sponsor: name, address, tax code or ID number, signature and seal.

Five mistakes we see most

  • Old template used after 1 July 2026
  • Address that does not match the temporary residence declaration
  • Requested duration longer than the work permit allows
  • Company seal placed by someone not registered on form NA16
  • Filing in the last few days before expiry, leaving no room for supplementation
