Form NA1 is the Vietnam visa application form for foreigners. You use it when applying for a visa at a Vietnamese embassy or consulate abroad, or when collecting a visa on arrival at an international border gate with an approval letter. The current version was issued with Circular 70/2026/TT-BCA and applies from 1 July 2026.
Who uses this form?
- Foreigners applying for a visa at a Vietnamese embassy or consulate abroad.
- Foreigners collecting a visa on arrival at an airport or international border gate with an approval (entry) letter – hand it in at the Landing Visa counter.
- Not for the e-visa: the e-visa is completed online at evisa.gov.vn.

Which language to fill in: This is the official bilingual Vietnamese–English form. Fill it in in English or Vietnamese, in CAPITAL LETTERS, with names exactly as in the passport. Do not translate the form into another language – only the official form is accepted.

Supporting documents and where to submit
- One form per person, together with a valid passport.
- Two 4×6 cm photos, white background, facing straight, no hat, no glasses (one glued on the form, one loose).
- Submit at the Vietnamese embassy/consulate abroad or at the visa counter at the border gate; the visa stamp fee is paid in cash (USD).
Common mistakes that get applications returned
- Names not in the same order as in the passport.
- Photo not 4×6 cm or background not white.
- Requested visa validity differs from the approval letter.
- Forgetting to sign at the bottom of the form.
