How to Fill In the Consular Legalisation Application Form

The application form for consular legalisation looks simple, and is refused for small reasons. Here is how to complete it.

Fields

Applicant: full name, ID or passport number, address and phone. If an agent files, name the agent and attach the authorisation.
Document details: name of the document, issuing authority, number and date of issue — copied exactly from the document, not summarised.
Country of issue and country of use: these decide which route applies.
Purpose: for example work permit, marriage registration, residence card, study.
Number of copies requested — ask for one more than you think you need; a second round costs another week.
Signature of the applicant or the authorised agent.
